Non-surgical Endodontic Retreatment Strategies

Endodontic treatment is a well-tested procedure that has provided pain relief and restored function and esthetics to patients for many decades. The ultimate biologic aim of endodontic therapy is preventing periradicular disease and promoting healing when the disease is already established. Although initial root canal treatment enjoys high success rates, it may not always result in the healing or prevention of apical periodontitis. Such cases of non-resolving post-treatment periapical disease are also termed endodontic failures. It is generally accepted that most failures occur when initial treatment procedures fail to reach adequate standards of infection control. If infection control fails during initial root canal treatment procedures, how predictable can it get during retreatment? Achieving infection control compatible with periradicular tissue healing can be quite challenging during orthograde retreatment, especially if the canal system is blocked. This lecture is designed to discuss the limits and possibilities of orthograde retreatment procedures.
